This example Custom Process is designed for practices using Forms.

This custom process sends out a summary notification to the relevant Owners of Forms when the form has been submitted by a client. A comment is also added to the submitted form for the Client Manager to alert them. The notification and comment can be altered to notify the relative Manager and/or Partner for the client by updating the merge fields.
