The MTD details confirmation form provides a template for practices to collect and confirm client information relevant to Making Tax Digital (MTD), helping practices prepare for clients' MTD requirements.

The form covers existing client details, income sources, sole trader income, record-keeping arrangements, quarterly update preferences, and HMRC agent authorisation information.

Download the Form template and import it into FYI. You can then customise the Form to suit your practice's requirements and send it to clients to review, complete, and submit their information.

What's included in the MTD details confirmation form?

The MTD details confirmation form is organised into five main sections, allowing practices to confirm client information and collect additional details relevant to MTD. Expand the sections below for more information.

Current Details

Display the client's existing contact information for review and confirmation, including:

  • First Name and Last Name.
  • Phone number.
  • Email address.
  • Postal address.

Clients can confirm whether their existing details are correct or provide updated information where changes are required.

Practices should review any changes submitted by the client and update the relevant client details in FYI as required.

Income Sources

Collect information about the client's income sources, including whether they receive income from self-employment.

The Form also collects information about the amount of self-employment income received.

This information helps practices understand the client's income circumstances when reviewing their MTD requirements.

Sole Trader

Collect updated information about the client's sole trader income.

Clients can indicate whether their sole trader income has changed significantly and provide their new expected annual income, where applicable.

Practices can use this information to review the client's circumstances and determine whether further information or action is required.

Your Records

Collect information about how the client currently maintains their financial records, including:

  • How income and expenses are recorded.
  • The software used to maintain their records.
  • Whether figures are manually retyped or transferred between systems.
  • Bookkeeper contact details, where applicable.
  • How up to date their records are.
  • When updated records can be provided to the practice.

This information helps practices understand the client's current record-keeping arrangements and identify areas that may require further review when preparing for MTD.

Authorisation Details

Collect information about the client's preferences and arrangements for MTD, including:

  • Their preferred approach to submitting quarterly updates.
  • Information relating to HMRC agent authorisation.
  • Additional comments or information relevant to their MTD arrangements.
  • Confirmation that the information provided is correct.

Practices should review the responses to confirm the client's preferences and identify any further steps required for HMRC agent authorisation or quarterly reporting.

Completing this section doesn't automatically authorise the practice with HMRC or submit quarterly updates.

Importing the MTD details confirmation form

Download the MTD details confirmation form using the file attached at the bottom of this article.

Once you've downloaded the file, import the Form template into FYI. Refer to Managing form templates for instructions on importing a Form template.

What's next?

After importing the MTD details confirmation form, review the template and customise it to suit your practice's requirements.

Review the existing client information fields, income questions, record-keeping options, quarterly update preferences, and HMRC agent authorisation questions to ensure they reflect the information your practice needs to collect.

Reviewing the Form template

Review any fields configured to display existing information from FYI and confirm that the relevant client details are available and correct before sending the Form:

  • Check the Form's questions and response options to ensure they are suitable for your practice's MTD procedures. Review any changes to MTD requirements separately, as the Form doesn't determine whether a client is required to comply with MTD.

For more information on editing Form fields and configuration, refer to Creating templates for Forms.

When you've finished reviewing and customising the template, ensure the Filing Status is set to Active. Refer to Managing form templates for instructions on updating the template.

Reviewing Form Rules

This Form uses Rules to dynamically display or hide fields and panels based on the information entered by the user. After importing the Form template, review the Rules in the Form Template Builder to ensure they're configured correctly for your practice. Follow the instructions in Creating a dynamic form using rules to review and configure the Rules, then test the Form to confirm the relevant fields and panels are displayed based on the responses selected.

Creating and sending the Form to clients

Once the template is ready, create a Client Form using the MTD details confirmation template. Select the relevant client and review the information populated from FYI before sending the Form for completion.

Refer to Creating client forms for instructions on creating a Client Form.

Send the Client Form to the client using the supported sharing options in FYI. The client can then review their existing information, provide additional details, and submit the completed Form.

For more information on sending Client Forms for external completion, refer to Creating client forms.

Reviewing submitted information

Completing the MTD details confirmation Form doesn't automatically determine a client's MTD obligations, establish HMRC agent authorisation, or submit quarterly updates to HMRC. Practices must complete any additional steps required as part of their MTD procedures.

When the client submits the Form, the Client Form in FYI is updated with their responses and remains filed against the client record selected when the Form was created.

Review the submitted information, including any corrections to the client's contact details, income information, record-keeping arrangements, quarterly update preferences, and HMRC agent authorisation details.

Where changes to client details have been requested, check whether the corresponding client information in FYI needs to be updated.

Use the responses to support your practice's review of the client's MTD requirements and any follow-up activities.
