Plan: Not Available: Intermediate, Pro Available: Elite

Users: Not Available: All users Available: FYI Admins or User Group permissions

Practice Management Source: Available: All sources

Issue

When you open a Form Template, the following warning displays at the top of the Form Builder:

This Form requires the following custom fields: (Custom Field Name)

Cause

A Custom Field added to the Form Template has been deleted from Practice Settings.

Solution

Use one of the following options to resolve the warning:

Recreate the Custom Field in Practice Settings with the same Name and Type, then refresh the Form Template, or
Remove the Custom Field from the Form Template.
