The Employee Performance Review (Sample) Form Template is designed to gather clear, structured information on how team members are progressing. It offers a structured way to record achievements, identify development areas and set future goals. The template can be sent directly to team members via email or built into a process automation so that reviews can be scheduled automatically.

When the Forms feature is enabled, the sample templates will be deployed and available from the Templates Workspace.

This article explains how the sample form is structured. We recommend taking a copy of the template, if you would like to customise the form.

Opening the Form Builder

The template will be imported with a Status "Draft". To open the template form builder:

  1. Navigate to Knowledge - Templates workspace.
     
  2. Select the "All" view to display all Active and Draft templates
     
  3. Search for Employee Performance Review and select the form template.

  4. Click the Edit option.
     
  5. The Form Template Builder window will open in a new Browser tab.

Template Layout and Fields

The Employee Performance Review (Sample) Form template is built using the template type "Wizard" which creates pages. Within these pages a combination of Headings, Images, Labels, Panels, FYI fields and Text Boxes are used.

Pages

The sample form contains 7 different pages containing different questions and information on each page:

  • Employee Details: Consists of an overview of the information contained in the form, explaining the structure and required fields such as Employee, Position and Manager. 
  • Core Responsibilities: Displays Employee and Manager evaluation questions to complete, displayed using Panels and Text Areas. Radio groups are used to gather ratings.
  • Goals and Achievements: Displays Employee and Manager evaluation questions to complete, displayed using Panels and Text Areas. Radio groups are used to gather ratings.
  • Strengths and Development: Displays Employee and Manager evaluation questions to complete, displayed using Panels and Text Areas.
  • Future Goals and Career Development: Displays Employee and Manager evaluation questions to complete, displayed using Panels and Text Areas.
  • Employee Feedback to Practice: Displays Employee and Manager evaluation questions to complete, displayed using Panels and Text Areas.
  • Acknowledgement: Consists of two checkboxes for both the Employee and Manager to complete, to confirm that the information provided is accurate, and the evaluation has been reviewed.

Other Layouts and Fields

A combination of different Field types and layouts are used to present the form in a structured layout:

Layout or Field Name Details
Heading

Provide a clear title for each of the pages of the form.

For example, the title of the Employee Details page is Performance Review - Employee Details.

Image

Displays the company logo to assist with branding.

The FYI logo has been used to provide an example on the first page.

Label

Assists in guiding clients with additional information when completing the form.

Labels are used for the following purposes:

  • Current Details
  • Updated Details
  • Form Description, displayed under the heading.
  • Submit information, which is displayed at the bottom of the article.
FYI Standard Fields Displays existing client information. All FYI Fields are displayed on the left-hand side of each panel. 
Text Box

Text boxes are used in this template to:

  • Capture updated information, displayed in each panel.
  • Allow the client to provide more information in the Other Notes text field displayed at the bottom of the form.
Notes/Help editor Notes/Help editor is used on various pages to provide the employee with additional information.
Radio Group

Radio Groups are used to quickly select an option form.

Radio Groups have been added to various pages of this form, to select Ratings. 

Checkbox

Checkboxes are used to quickly select or confirm an option in the form.

A checkbox has been added to the Acknowledgement page. This is a mandatory checkbox, meaning Employees must agree that they must confirm the information provided is accurate.
