Forms Branding allows practices to define their primary and secondary colour, and apply a form header that includes their practice logo, when creating forms.
Once the Forms feature has been enabled by an FYI Admin, practices can update their branding from Practice Settings.
The Branding settings are specific to Forms and do not affect other documents in FYI.
Default Form Brand Colours
Forms Branding allows practices to personalise their forms. When forms are enabled, all forms and form templates will default to the Default Primary and Secondary Brand Colour.
When an FYI Admin updates the branding, the colour and heading will be applied to the following forms:
- All active forms, including General Forms and Submitted Client Forms.
- All new Client, Internal and General Forms.
- All existing and new Form Templates.
- When new Forms are converted to PDF.
Primary Brand Colour
The following table describes the form fields, their default primary colour and how each element in a form applies the Primary Brand colour.

Secondary Brand Colour
The following table describes the form fields, their default secondary colour and how each element in a form applies to the Secondary Brand colour.

Setting the Branding Settings
When Forms is enabled, all sample and new forms will use the primary and secondary branding colours. To display your practice's branding colours:
- From FYI, click Settings - Practice Settings.
- Select Practice settings, and from the drop-down select Forms.

The Forms setting page will be displayed. Click the Branding tab.
- The Brand settings will be displayed.
- Update the branding colours and enable a form header.
Updating the Primary and Secondary colours
There are two ways that the Primary and Secondary Brand Colours can be updated:
- Hexadecimal Colour Code: for practices that know their hex code, this can be entered.
- Colour Picker Window: allows practices to determine their primary colour visually or enter the RGB code.
To update your Primary and Secondary Brand Colours using a hexadecimal colour code:

From the Branding tab, enter your primary or secondary colour's hexadecimal code.
- Click Apply.
- A Message will display to confirm the changes. Click Yes to apply the changes to all forms. Click No to return to the Branding settings.
- A confirmation message will display to confirm that the brand colours have been updated successfully.
Once applied, all active forms, including submitted forms, will be updated with the changes to the respective brand colours. Any form templates, new and existing, will also be updated.
If you don't know your primary and secondary colour hexadecimal codes, you can update your brand colours from the Colour Picker Window.

From the Branding tab, click the colour swatch corresponding to the Primary or Secondary Brand Colour.
- The Colour Picker Window will open.
- From the window, select a colour using one of these options:
- type in the corresponding RGB (Red, Green, Blue) values,
- move your cursor around to find the colour. When you move the cursor, the picker updates and displays an example of the colour, along with the RGB code, or
- click the chevron icon to change between RGB, HSL (Hue, Saturation, Lightness) or Hex values. Enter the corresponding code, where required.
In the following example, the chevron has been clicked to display the HSL values.
- Click Apply.
- A Message will display to confirm the changes. Click Yes to apply the changes to all forms. Click No to return to the Branding settings.
- A confirmation message will display to confirm that the brand colours have been updated successfully.
Once applied, all active forms, including submitted forms, will be updated with the changes to the respective brand colours. Any form templates, new and existing, will also be updated.
Enable a Form Header
A Form Header is the branded section displayed at the top of a form. It can be used to apply your practice’s branding, such as your practice name and logo, to all created forms.
The Form Header is displayed when the form is viewed externally, including when it is viewed by a client. It is not displayed in the Preview pane in FYI.
Once enabled, to preview the Form Header as it will appear to a client, click Open External from the Preview toolbar. The form will open in a new browser tab with the Form Header displayed.
The background colour of the form header is set to your Primary colour. By default, the Form Header setting is disabled.
Before enabling a form header, update the form header fields to suit your practice:

Configure the Form Header settings to suit your practice branding:
Form Header Fields Details Upload logo Upload your practice logo to display on the left of the Form Header and in the browser tab when a form is opened.
To remove a form logo, click the x icon.
Supported file formats are PNG, JPG, and SVG.
Branding text Displays your practice name by default.
Update the text to display alongside the logo in the Form Header, where required.
Heading alignment Select how the logo and Branding text are aligned in the Form Header. - Review the preview displayed below the form heading settings.

Enable the form header settings. Once enabled, these changes will automatically be applied to all forms, including existing forms.
All changes to form header settings are displayed in Practice Activity.
Resetting Default Brand Colours
Once you have set your Primary and secondary colours, you can reset this at any time to the Default Brand Colours.
- Navigate to Forms Settings - Branding tab.
- Select Reset.
- A confirmation message will display, confirming that the brand colour/s will be reset to the default brand colour.
- Click Yes to reset all forms to the default colour. Click No to return to the Branding settings.
- A confirmation message will display to confirm the brand colour has been updated successfully.
Examples of Branding Setting Changes
The following example compares the Client Satisfaction (Sample) form using the Default Brand colours with the Form Header disabled, and the same form with the Default Brand colours and Form Header enabled.
