The Professional Development Request (Sample) Form Template streamlines the process of employees applying for training or development opportunities. The form allows employees to submit key details including the course or program, duration and associated costs.
When the Forms feature is enabled, the sample templates will be deployed and available from the Templates Workspace.
This article explains how the sample form is structured. We recommend taking a copy of the template, if you would like to customise the form.

Opening the Form Builder
The template will be imported with a Status "Draft". To open the template form builder:
- Navigate to Knowledge - Templates workspace.
- Select the "All" view to display all Active and Draft templates.
- Search for Professional Development Request (Sample) and select the form template.
- Click the Edit option.
- The Form Template Builder window will open in a new Browser tab.
Template Layout and Fields
The Professional Development Request (Sample) Form template is built using a combination of Headings, Images, Labels, Panels, FYI fields, Advanced Fields, Text Boxes, Dropdown lists and Text Area.

Panels
The sample form contains five Panels:
- Employee Details: Allows employees to complete their details, including Name, Job Title and their Manager's details. The Name and Position field is marked as Required, to ensure that this information is captured when completing the form. Both the Name and Manager fields are FYI fields, displaying as drop-downs when the form is being completed.

Training/Course Details: Includes details such as Course Name, the Provider and Dates, including duration. Drop-down fields are used to distinguish between the Type of course and the delivery method. The Training/Course Name field is marked as Required, to ensure this information is captured.
- Reason for Request: This panel enables the employee to provide a more detailed explanation, in their own words, of why they wish to attend the training . Using text areas allows the employee to freely answer the questions without being constrained to a one-line answer. The first question is marked as Mandatory.

Costs: Employees can add the estimated costs of the training and how it will be covered to assist with their request.

Employee Acknowledgement: Having an acknowledgment panel allows an employee to confirm that they understand the approval requirements. The tick box acts as an easy way to acknowledge their understanding. The employee must tick this box to be able to submit the form.
Other Layouts and Fields
A combination of different Field types and layouts are used to present the form in a structured layout:

Displays the company logo to assist with branding. The FYI logo has been used to provide an example. |
| Label |
Assists in guiding clients with additional information when completing the form. Labels are used for the following purposes:
Tooltips have been added to certain labels to assist with further information. |
| FYI Fields: User List |
Used to capture the FYI User details. The User list will display in a drop-down for both the Name and Manager fields of the Employee Details panel. |
| Text Box |
Text boxes are used in this template to capture short answers regarding the request, displayed in each panel. The Estimated Cost displayed in the Costs panel, includes a $ prefix in the Text Box Component settings. |
| Dropdown List |
Allows the employee to select from a set drop-down list of answers. Dropdown lists have been used in both the Training/Course Details and Costs panels. |
| Advanced Fields: Upload |
Allows the employee to upload supporting documentation, such as quotes or brochures. This field is displayed in the Costs panel. |
| Date Time | Used to capture the duration of the Course or Training that is being requested. |
| Text Area |
Text area allows multi-line responses, designed for longer, free-form responses. Text area has been used within the Reason for Request panel, allowing employees to provide more information on their request. |
| Checkbox |
Checkboxes are used to quickly select or confirm an option in the form. A checkbox has been added to the Employee Acknowledgement panel. This is a mandatory checkbox, meaning Employees must agree that they understand the Approval Conditions. |
