The Client Satisfaction (Sample) Form Template is designed to request feedback from clients following the completion of a job or service. The template provides a set of rating-scale questions across key services. The template can be sent directly to clients via email or built into a process automation so that requests for updated details can be scheduled automatically.
When the Forms feature is enabled, the sample templates will be deployed and available from the Templates Workspace.
This article explains how the sample form is structured. We recommend taking a copy of the template, if you would like to customise the form.
Opening the Form Builder
The template will be imported with a Status "Draft". To open the template form builder:
- Navigate to Knowledge - Templates workspace.
- Select the "All" view to display all Active and Draft templates.
- Search for Client Satisfaction (Sample) and select the form template.
- Click the Edit option.
- The Form Template Builder window will open in a new Browser tab.
Template Layout and Fields
The Client Satisfaction (Sample) Form template is built using a combination of Headings, Images, Labels, Panels, Text Areas and Summary Table.

Panels
The sample form is structured with a single panel, titled Survey, which combines various Form fields. This panel incorporates Radio Group questions and Headings, along with a Text Area and a Summary table to create a comprehensive feedback form.
Within the Survey panel, three key sections are included:
- Overall Experience: A mandatory rating question designed to capture the client's overall level of satisfaction. The Radio group settings are configured as "Required" to ensure a response is provided before the form is submitted.
- Service Dimensions: Contains a series of five rating questions covering Communication, Responsiveness, Professionalism, Quality of Work and Value for Money.
- Open Feedback: A text area field that provides space for clients to share additional comments or suggestions.
Summary Table
The Summary Table displays the key responses from completing the questions on the form in a clear, organised format, with responses presented in rows and columns. Once a client completes the form, their answers are displayed and can be refreshed to show the latest answers.
When viewing the form template builder, the table will appear empty, however, when selecting preview, the columns Summary and Answer are displayed.
Other Layouts and Fields
A combination of different Field types and layouts are used to present the form in a structured layout:
