The Update Client Details (Sample) Form Template is designed to request updated information from your clients. The template can be sent directly to clients via email or built into a process automation so that requests for updated details can be scheduled automatically.
When the Forms feature is enabled, the sample templates will be deployed and available from the Templates Workspace.
This article explains how the sample form is structured. We recommend taking a copy of the template, if you would like to customise the form.
Opening the Form Builder
The template will be imported with a Status "Draft". To open the template form builder:
- Navigate to Knowledge - Templates workspace.
- Select the "All" view to display all Active and Draft templates.

Search for Update Client Details (Sample v1.0) and select the form template.
- Click the Edit option.
- The Form Template Builder window will open in a new Browser tab.
Template Layout and Fields
The Update Client Details (Sample) Form template is built using a combination of Headings, Images, Labels, Panels, FYI fields and Text Boxes.

Panels
The sample form contains three Panels: Client Details, Physical Address and Postal Address. Each panel follows the same structure, combining labels, FYI fields and text boxes to easily display relevant information.
Each panel contains two sections:

Current Details: The left-hand side displays FYI fields selected from the FYI Standard Fields Component. In the field settings, each of these fields is configured as read-only to ensure that the client's current information is displayed for reference but cannot be changed.
- Updated Details: The right-hand side displays Text Boxes, which are editable fields where clients can enter new or corrected information when completing the form. The Updated Details label includes a Tooltip in the field settings to offer extra guidance to the client. Hovering over the tooltip, the message 'Only add information if it has changed.' will display.

Other Layouts and Fields
A combination of different Field types and layouts are used to present the form in a structured layout:
